District Nurses


District Nurses

Due to a change in the district nursing organisation - we now have a team of new nurses.

We understand this may be distressing for some patients but apologise on behalf of their organisation for the inconvenience.

All distrist nurse enquiries should be dealt with by the district nurses directly.  There is an office administrator working in the district nurse office Monday-Friday 8.30 - 12.30, she is able to take all enquiries regarding the nursing team and patient visits.

INR Machine



The surgery has invested in a new anticoagulation machine.  This means patients that require their INR testing can now have it done at the surgery via a fingerprick test and get the results the same day!


Some patients may still be required to have a full blood sample taken - The nurse will be able to advise you on this.


So far we have had fantastic results and many patients have been very pleased with the new quick and easy service.

We Won an Award!


Quality Practice Award

It is with pleasure that the Chantry Health Group wishes to announce that it has achieved the Quality Practice Award.

The Quality Practice Award (QPA) is a criterion-based quality accreditation process undertaken by Primary Health Care Teams across the United Kingdom.

The purpose of the award is to improve patient care by encouraging and supporting practices to deliver the very highest quality care to their patients.

The Quality Practice Award recognises the commitment of the entire team, including the attached staff, in providing high quality care for patients within a learning and adaptive environment.

QPA is the highest attainable award available from the royal college of general practitioners, it  requires more in the way of assessment of patient  experience, it has a large clinical component and asks the contributing team  members to demonstrate reflective learning. All of the QPA criteria must be met to achieve the award. 


Changes to Prescriptions


Repeat prescription requests take up to 48 hours to process. Please allow yourself enough time to order your medication before running out.


How to Order Online - THIS HAS NOW CHANGED.   To register for online prescription services and online appointment services ask a receptionist for your unique log in details.  N.B. Please allow 24 hours after gaining your password before trying to log into the online services.


Changes - Medication Reviews.  Your medication reviews have now changed.  When your review is due you will receive a phone call or letter advising you to book in with the relevant health professional.  Please do not book a review on the advise of your prescription.

Text Reminders

From March 1st we will be running a text reminder service to all patients who have mobile telephone numbers registered on their file.

The way it will work:

1. Register your mobile number with reception.

2. You will receive a text reminder at least 12 hours before your next appointment is due.

3. If you are unable to attend your appointment you can ring and cancel!

4. If you do not wish to receive appointment reminders via text message then please request to opt-out at reception.

Are you a carer?

If you provide UNPAID support and assistance for a relative, friend or neighbour, you are a carer.

If you provide UNPAID care for a relative, friend or neighbour who can not
manage daily tasks, you are a carer.

If you or the person you care for are registered at this surgery please let us know!  We can put you in-touch with a great carers support group that help you to plan and manage care as well as provide extra support for the person you care for.

N.B. The carer and the person cared for will need to sign a consent form for information to be given to the George Hardwick Foundation, Victoria Street, Grimsby.
