Practice information
Church View Health Centre is a purpose built primary care centre which opened on 18 October 2004. The facilities are shared between two practices, ourselves and The Pelham Medical Group. This was built under the private finance initiative.
The Chantry Health Group evolved from a long series of partnerships which were based at 17 Chantry Lane, Grimsby, where GPs provided services for over 50 years since the inception of the NHS.
The practice is committed to high class, evidence based services and providing as many services as possible within our purpose built centre. We are also committed to developing our people so that we provide the best service possible within the resources available, and as such achieved the Investors in People award in 2010. In 2012 as a practice we acheived a QPA (Quality Practice) award.

To enhance the service provided to patients/customers by making best use of resources and development of skills of all staff whilst developing our training provision. Our patients will be treated as people not just a medical condition. This means we plan care, which emphasises the patient's individual needs, dignity and strict confidentiality.
The care given will be research based and delivered with the highest standards. Advice, support and information will be available in order to help our patients make the right choices. We will set standards of care and review them periodically in the form of surveys and requests for feedback and we will act upon these results accordingly.
